Web form for creating the self-disclosure document

If you would like to apply for one of the Master's programs in Business Informatics, Computer Science or Data Science, you must complete the following web form. At the end of the form, you can create a PDF document that you must submit with your application. Please make sure you read all the information texts, notes and descriptions provided throughout the web form.

Technical information:

The web-form is designed to be used from a PC and filling it in on a mobile device may not be convenient. If you still want to use a mobile device, use one with a screen as large as possible and use the device in landscape mode.

In case you experience technical problems with the web form, here are some troubleshooting hints:

  • Opening the web form for the first time can take a while. If nothing happens after a few seconds, you should try to reload the page.
  • You should also make sure to use a recent version of your browser; the web form has been tested with Chrome, Firefox, Edge and Safari.
  • If the web form becomes unusable after an input from you, you can try to press the “reset all” button and avoid the erroneous input. If that is not possible, try to delete your browser data and reload the page.
  • If the web-form still fails, you can send an error report. If the web form is still sufficiently responding, press the “Report Error” button at the bottom of the page and press the “Download State” button in the dialog, which downloads a file that you should attach to your report. Send a description of the error and your actions that led to it to the email address , possibly including a screenshot.
